I agree to 3 monthly payments of £200.

Payment 1 will be paid on submission of this form.

Payment 2 will automatically be taken 30 days from submission of this form.

Payment 3 will automatically be taken 30 days from payment 2.

If for any reason I have to stop payment, all training access will be withdrawn immediately.

I agree to pay the balance in full over a three month period.
